How to inform the State Data Protection Inspectorate about the Data protection officer of your organization

What information about designated Data protection officer your organization should provide:

Name of the controller or data processor, contact details
Information on whether data protection officer is designated by data controller or by data processor
Name and surname of DPO
If appointed DPO is an employee of data cotroller or data processor – the job title; if appointed DPO is an an employee of another legal entity – the name that legal entity
DPO‘s address, phone number, email address, other means of communication
